To create a Document/Image question type:
Select Document/Image from the Question Type menu
Enter the Question Text
Click on Upload Document or Image to upload
Select Browse file to locate your document or image
Click Save
Select Attributes
Click Preview to preview the question
Click Save to save and create your question
