To create a Upload Media question type:
- Select Upload Media from the Drop Down Question Type
- Type your question
- Select your preferred Attributes
- Choose to view Photo, Video or Document information (you can ask for a combination of media as well)
- Click on Preview to review your question
- Click on Save to save and create your question
